Restart Script

I've looked around and have found a few threads about this, none of which seem to work. I've searched before posting, maybe I'm just that much of a noob, but I'd appreciate the help if offered.

I can't seem to get the auto restart function to work properly. Maybe my startup script isn't correct, so if you have one that'd be great. Also how would I include it so it removes the current screen and creates a new screen session automatically?
